This webpage houses all of the Elauwit model notices along with your Employee Benefits Summary Plan Description for the 2017 benefit year that starts on October 1, 2017. Also, on this page are each carriers certificate of coverage for your benefit. We have also included summaries of these documents for your review along with some carrier forms below. Health Insurance

Our health plan will continue with Blue Cross Blue Shield of SC. We have two health plans for you to chose from and the information on each plan is presented below.

Dental Insurance

Our dental plan is now with Metropolitan Life. Our benefits cover in network and out of network benefits. We have provided a provider link below to locate an in network provider for your convenience.

Life & Disability Insurance

Our life insurance and disability coverage will be covered by Metlife. Our information and any forms you may need are below. We have an open enrollment at this time and you will be able to purchase all coverage’s without health questions*.

Vision Insurance

Our vision coverage is now with Metlife. The information is provided below as well as a link to the provider network.

BlueCross & BlueShield My Health Toolkit

As an employee of Elauwit, take charge of your health and start using the My Health Toolkit from BlueCross. Employee Assistance Plan (EAP)

Elauwit offers all of it’s employees access to an Employee Assistance Plan (EAP). Our EAP is administered by First Sun EAP. More information on the plan is provided below. For any other questions, see Human Resources.
